Terms & Conditions
These terms and conditions apply to all those (“Organizer”) who are creating a tribute webpage (in memory, in honour, or in celebration) in support of CAMH Foundation (the “Tribute Page”).
- I agree to provide CAMH Foundation with all the proceeds and donations of my Fundraising Page. I acknowledge that any expenses I incur are my responsibility and that CAMH Foundation is not responsible for any losses that I may incur in relation to the Fundraising Page. I will clearly identify to donors that the proceeds and donations will be given to CAMH Foundation.
- "CAMH", "CAMH Foundation", and related logos are trademarks of CAMH Foundation and CAMH. Such trademarks cannot be used without the prior written approval of CAMH Foundation. Organizers may use “In support of CAMH” logo on their promotional materials.
- I acknowledge and agree that CAMH Foundation and CAMH, their employees, directors, officers, members, agents, successors or assigns (collectively the "Indemnified Parties"), shall not be held liable for any injury, loss or damage, however caused, which the Indemnified Parties may incur resulting from or arising out of the Tribute Page.
- Tax receipts will be issued in compliance with CRA guidelines, as follows:
- CAMH Foundation will issue official charitable tax receipts for all individual donations of $10 or more made directly to CAMH Foundation online or by cheque, and for donations under $10 upon request.
- Business acknowledgment letters will be issued for eligible corporate contributions.
- I agree to respect the confidentiality of personal information that may be provided to me through the Tribute Page and acknowledge and agree to the following:
- I will only use personal information to administer my Tribute Page in support of CAMH Foundation.
- I agree that all such personal information is collected on behalf of CAMH Foundation.
- Personal information received through the Tribute Page will be kept secure at all times (whether stored in physical form or on a computer); access to and disclosure of personal information shall be limited to myself (as the Tribute Page organizer) and CAMH Foundation employees only.
- All donor data remains the exclusive property of CAMH Foundation. To learn more about commitments to donors by the CAMH Foundation you can read the donor bill of rights and our CAMHF Privacy statements
- These Terms and Conditions are in addition to the applicable policies of CAMH Foundation and to the Terms of Use on the CAMH Foundation website. Please note that the Foundation reserves the right, at its discretion, to change, modify, add, or remove portions of its policies or Terms of Use or these Terms and Conditions.
Agreement
These Terms & Conditions shall be governed by the laws of the Province of Ontario and the laws of Canada applicable therein. All disputes shall be submitted to the exclusive jurisdiction of Ontario courts.
By accepting, the Organizer certifies that:
- They have read, understood, and agree to abide by all policies and guidelines outlined above.
- All publicity, promotional materials, and media communications will be approved by CAMH Foundation before release.
- They acknowledge the event is fully their responsibility and that CAMH Foundation is not liable for any expenses, actions, or outcomes related to the event.
